You know what’s kind of wild? We can identify products by their container designs. You’d know a ketchup bottle, even if it was empty and unlabeled, no matter what the brand. You’d know a pickle jar, or a milk jug, or a bottle of salad dressing, or a cereal box just by their container shapes.




Same goes for the big software categories. You’d know a spreadsheet anywhere — formula bar at the top, grid below — no matter what company made it. Or e-mail program, word processor, Web browser. They all work pretty much alike.


But there’s one software category, an incredibly important one, where there’s no standard design or set of features: calendar software. Each one seems to have evolved on its own Galápagos island.


Take the new Calendar app in Windows 8. So much of Windows 8’s touch-screen mode is modern, updated and fresh — color, gestures, typography — that you’d expect an equally modernized calendar app at its heart.


Wow, would you be wrong. Listen, Microsoft: 1990 called. It wants its calendar back.


You can’t drag vertically through the Day-view column to create an appointment. You can’t drag an appointment to reschedule it. You can’t record an auto-repeating appointment like “Monday, Wednesday, Friday” or “first Tuesday of the month.”


And incredibly, you can’t create separate categories, like Home, Work and Social. There’s no way to color-code your appointments or hide certain categories.


That same week, on another computer, I installed a Mac calendar program called BusyCal 2.0. You know what’s so brilliant? When you open it, today’s date is always in the top row, no matter what week of the month this is. You always see the next four or five weeks, even if some are in the following month.


And why not? Almost always, you open your calendar to check coming dates — so why fill the screen with dates that have already gone by? That’s a limitation of paper calendars, where every month shows 1 in the first square.


And that’s when I had my epiphany. Our electronics are capable of fantastic flexibility, features and design; why are we still modeling our digital calendars on paper ones?


Apple’s Calendar app for the Mac goes so far as to display a little leather “binding” at the top, complete with scraps of torn-off “paper” to indicate where previous months’ “pages” have been torn off. Why?


If you spend enough time with the world’s calendar apps, you can see, through the mist, a vision of the ultimate digital calendar program. If you could mix and match the best of all the motley calendar apps, here’s what you might come up with.


¶ Give us an alternative to tabbing from Start Time to End Time and typing numbers into a tiny New Event box. Let us drag to indicate a meeting’s length. Or give us speech — intelligent speech, like Siri on the iPhone. “Make an appointment next Tuesday morning at seven: tennis with Casey,” you can say. Your hands never leave the wheel, the cat or the delicious beverage.


We should also be able to type plain-English phrases like “tomorrow 1pm lunch mtg” or “4/15 730p Dinner with boss,” and marvel as it creates the right appointment on the right calendar square at the right time. (Google, Apple’s Calendar for the Mac, BusyCal and, in particular, the iPhone app Fantastical can all do this.) Here again, you’re not fiddling with a dialogue box to enter a new event.


¶ Microsoft’s greatest calendaring effort remains Outlook, the e-mail program that comes with some versions of Microsoft Office. Outlook has its detractors, but one thing it got right is integration with your e-mail and address book. What are appointments, after all, but interactions with people you know — and how better to set up meetings with them than with e-mail?

“The moment I feel like I can move, I will go back to the mountains, rearm myself and fight you again,” Mr. Mohammed, who is recovering from a recent gunshot wound to the leg caused by a clash with a police officer, recalled warning the governor, police chief and top security official of Badghis Province.


He is not alone. Interviews with more than a dozen former insurgents find a group embittered and torn about their choice to lay down their arms. Many are unable to work and often unable to return to their villages for safety reasons; most feel the government has cheated them.


“I am just counting the days before someone kills me,” said Akhtar Mohammad, a former Taliban commander in Oruzgan. “Every passing moment I feel regret for joining the peace process.”


Mr. Mohammad, who is from the south, where the insurgency is the deadliest, said the only thing he had to show for his decision to switch sides was the turban he had been given for his reintegration ceremony.


Two years into one of the most ambitious efforts to lure Taliban fighters off the battlefield, many Afghan and Western officials acknowledge that the results have been disappointing. Though the number of people claiming the amnesty has swelled in the past year, violence remains high in the hotbeds of the insurgency, where the program has struggled to take hold.


Of the roughly 6,100 participants so far, 80 percent come from the relatively peaceful north and west of the country, where the Taliban presence is thinnest. And even some of those fighters say their activity was more criminal than political or ideological, even though the program was designed to flip the most committed fighters.


Many onetime fighters say they were only loosely affiliated with the Taliban. Though widely thought of as a unified Islamic movement, the Taliban in reality consist of a shifting constellation of forces whose agendas sometimes line up. Many of the insurgents who have given up fighting said they were formerly of Hezb-e-Islami, a rival faction to the Taliban that has been losing power and influence in recent years.


Although some fighters have found employment, Western officials argue that the reintegration initiative was never meant to be a jobs program. It pays a modest stipend, about $100 to $200 monthly for the first three months, depending on the fighter’s rank. In theory, the former fighters can find work on development projects financed in their home villages, though many projects have failed to get off the ground.


In two years, the so-called High Peace Council has spent less than half of what officials hoped to spend in 2011 alone. Tight controls have stalled village development projects meant to employ the former insurgents in their communities. About $180 million of the $235 million the program was granted sits idle in Afghan coffers.


The officials running the program called it a work in progress that would improve over time. The importance of employment for former fighters is acknowledged as an important element of keeping them from picking up arms again.


“We’re working on ways to improve the effectiveness of the program,” said Paul Martin Mason, who oversees the reintegration program for the United Nations Development Program.


In the meantime, many former fighters are expressing second thoughts with their decisions. Having tired of endless fighting, Hazrat Mir, a former Taliban commander in Laghman, joined the government with 130 of his men last year.


With promises of jobs and land, Mr. Mir said, his men left their villages and turned in their weapons, a dangerous prospect given the threat from former comrades. The danger, it turned out, was made clear by an attack the day before their reintegration ceremony in which the Taliban killed Mr. Mir’s top deputy.

Safety of Boeing 787 Dreamliner Called Into Question







NEW YORK — For Boeing, much rides on the success of its newest and most sophisticated jet, the 787 Dreamliner. But a spate of mishaps is reviving concerns about the plane’s reliability and safety.




The plane had a new problem Wednesday when the Japanese carrier All Nippon Airways cancelled a domestic flight after a computer on board erroneously showed problems with the aircraft’s brakes. A spokeswoman for the airline, Megumi Tezuka, said the computer glitch was similar to one that appeared when the carrier first started flying the Dreamliners in 2011.


The flight NH698 had been due to depart from Yamaguchi Ube airport in southern Japan for Tokyo’s Haneda airport at 4:50 p.m. local time. The flight’s 98 passengers were transferred to a later flight.


On Tuesday, a fuel leak forced a 787 to return to its gate minutes before taking off from Boston. On Monday, an electrical fire had broken out on another plane. Both of those incidents affected planes operated by Japan Airlines at Logan International Airport in Boston.


The three events were the latest in a series of problems with the 787, which entered commercial service in November 2011 and has had technical and electrical malfunctions since then. Boeing delivered 46 planes last year, more than any analyst had predicted, and has outlined ambitious plans to double its production rate to 10 planes a month by the end of this year.


Boeing expects to sell 5,000 of the planes in the next 20 years. The basic model has a list price of $206.8 million, but early customers typically received deep discounts to make up for the production delays and teething problems. All this means it could be years before Boeing starts recouping its investment costs and turning a profit on the planes.


Shares of Boeing dropped 2.6 percent to $74.13 Tuesday, extending the drop of 2 percent Monday.


The 787 makes extensive use of new technology, including a bigger reliance on electrical systems, and is built mostly out of lightweight carbon composite materials. While the problems so far do not point to serious design problems with the airplane, they represent an embarrassment to Boeing’s manufacturing ability.


“None of this is a showstopper, and none of this should signal this product is fundamentally flawed,” said Richard Aboulafia, an aerospace analyst at the Teal Group, a consulting firm. “But whether these are design glitches or manufacturing glitches, either way it’s a serious hit to Boeing’s image.”


The fuel leak Tuesday was spotted by another pilot as JAL Flight 007, bound for Tokyo, was taxiing and getting ready to take off, said Richard Walsh, a spokesman for the Massachusetts Port Authority. The plane was towed back to its gate and the leak of about 40 gallons, or about 150 liters, was cleaned up.


The flight with 178 passengers and 11 crew members, scheduled to take off at noon, eventually left Boston at 3:47 p.m.


An electrical fire Monday on a 787 was traced to a battery connected to the plane’s auxiliary power unit, which runs electrical systems when the plane is not getting power from its engines.


The fire broke out about 30 minutes after the flight landed from Tokyo, and all 183 passengers and crew members had left. The smoke was first detected in the cabin by maintenance and cleaning personnel who were on the parked plane and notified the airport’s fire department.


The National Transportation Safety Board, which is investigating the electrical fire, said the battery had “severe fire damage.”


New planes often experience problems in the first few years of production. But the succession of issues with the 787, which has already been marred by production delays of years, has revived concerns about the plane’s reliability and safety.


Last month, the Federal Aviation Administration ordered inspections of fuel line connectors on all 787s, warning of a risk of leaks and fires. Separately, a United Airlines 787 was also diverted in December after one of six electrical generators failed in flight.


In a statement Tuesday, Boeing said that it saw no relationship between the battery problem and previous incidents with the 787’s power system, which involved faults in power panels elsewhere in the electrical equipment bay.


“Boeing is cooperating with the N.T.S.B. in the investigation of this incident,” the company said. “Before providing more detail, we will give our technical teams the time they need to do a thorough job and ensure we are dealing with facts, not speculation.”


A Boeing spokesman, Marc Birtel, said the plane maker was aware of the fuel leak incident but declined to comment.


A spokeswoman for Japan Airlines in Tokyo said that the company was still gathering the details about the two incidents and that there were no plans to change its orders for 787s. The airline has seven 787s already in service, and 38 more on order. The spokeswoman declined to be named, citing company policy.


All Nippon Airways, which also operates Dreamliners, likewise said there were no plans to change its orders for the aircraft. Ms. Tezuka, the spokeswoman for ANA, said the airline had 17 787s in service and another 49 on order.


United Airlines, currently the only airline in the United States operating 787s, said it had performed inspections on all six of its 787s after the electrical fire Monday. It did not cancel any of its flights today, said Mary Ryan, a spokeswoman for the airline. She said United “continues to work closely with Boeing on the reliability of our 787s.”


But she declined to comment on a report in The Wall Street Journal that said the airline had found improperly installed wiring in components associated with the auxiliary power unit.

1 large or 2 small or medium cauliflowers, broken into small florets


1 5-ounce can water-packed light (not albacore) tuna, drained


1 plump garlic clove, minced or pureéd


1/3 cup chopped flat-leaf parsley


3 tablespoons capers, drained and rinsed


1 tablespoon fresh lemon juice


3 tablespoons sherry vinegar or champagne vinegar


6 tablespoons extra virgin olive oil


Salt and freshly ground pepper


1. Place the cauliflower in a steaming basket over 1 inch of boiling water, cover and steam 1 minute. Lift the lid for 15 seconds, then cover again and steam for 5 to 8 minutes, until tender. Refresh with cold water, then drain on paper towels.


2. In a large bowl, break up the tuna fish and add the cauliflower.


3. In a small bowl or measuring cup, mix together the garlic, parsley, capers, lemon juice, vinegar, and olive oil. Season generously with salt and pepper. Add the cauliflower and toss together. Marinate, stirring from time to time, for 30 minutes if possible before serving. Serve warm, cold, or at room temperature.


Yield: Serves 6 as a starter or side dish


Advance preparation: You can make this up to a day ahead, but omit the parsley until shortly before serving so that it doesn’t fade. It keeps well in the refrigerator for up to 5 days.

Online Banking Attacks Were Work of Iran, U.S. Officials Say





SAN FRANCISCO — The attackers hit one American bank after the next. As in so many previous attacks, dozens of online banking sites slowed, hiccupped or ground to a halt before recovering several minutes later.

But there was something disturbingly different about the wave of online attacks on American banks in recent weeks. Security researchers say that instead of exploiting individual computers, the attackers engineered networks of computers in data centers, transforming the online equivalent of a few yapping Chihuahuas into a pack of fire-breathing Godzillas.


The skill required to carry out attacks on this scale has convinced United States government officials and security researchers that they are the work of Iran, most likely in retaliation for economic sanctions and online attacks by the United States.


“There is no doubt within the U.S. government that Iran is behind these attacks,” said James A. Lewis, a former official in the State and Commerce Departments and a computer security expert at the Center for Strategic and International Studies in Washington.


Mr. Lewis said the amount of traffic flooding American banking sites was “multiple times” the amount that Russia directed at Estonia in a monthlong online assault in 2007 that nearly crippled the Baltic nation.


American officials have not offered any technical evidence to back up their claims, but computer security experts say the recent attacks showed a level of sophistication far beyond that of amateur hackers. Also, the hackers chose to pursue disruption, not money: another earmark of state-sponsored attacks, the experts said.


“The scale, the scope and the effectiveness of these attacks have been unprecedented,” said Carl Herberger, vice president of security solutions at Radware, a security firm that has been investigating the attacks on behalf of banks and cloud service providers. “There have never been this many financial institutions under this much duress.”


Since September, intruders have caused major disruptions to the online banking sites of Bank of America, Citigroup, Wells Fargo, U.S. Bancorp, PNC, Capital One, Fifth Third Bank, BB&T and HSBC.


They employed DDoS attacks, or distributed denial of service attacks, named because hackers deny customers service by directing large volumes of traffic to a site until it collapses. No bank accounts were breached and no customers’ money was taken.


By using data centers, the attackers are simply keeping up with the times. Companies and consumers are increasingly conducting their business over large-scale “clouds” of hundreds, even thousands, of networked computer servers.


These clouds are run by Amazon and Google, but also by many smaller players who commonly rent them to other companies. It appears the hackers remotely hijacked some of these clouds and used the computing power to take down American banking sites.


“There’s a sense now that attackers are crafting their own private clouds,” either by creating networks of individual machines or by stealing resources wholesale from poorly maintained corporate clouds, said John Kindervag, an analyst at Forrester Research.


How, exactly, attackers are hijacking data centers is still a mystery. Making matters more complex, they have simultaneously introduced another weapon: encrypted DDoS attacks.


Banks encrypt customers’ online transactions for security, but the encryption process consumes system resources. By flooding banking sites with encryption requests, attackers can further slow or cripple sites with fewer requests.


A hacker group calling itself Izz ad-Din al-Qassam Cyber Fighters has claimed in online posts that it was responsible for the attacks.


The group said it attacked the banks in retaliation for an anti-Islam video that mocked the Prophet Muhammad, and pledged to continue its campaign until the video was scrubbed from the Internet. It called the campaign Operation Ababil, a reference to a story in the Koran in which Allah sends swallows to defeat an army of elephants dispatched by the king of Yemen to attack Mecca in A.D. 571.


But American intelligence officials say the group is actually a cover for Iran. They claim Iran is waging the attacks in retaliation for Western economic sanctions and for a series of cyberattacks on its own systems. In the last three years, three sophisticated computer viruses — called Flame, Duqu and Stuxnet — have hit computers in Iran. The New York Times reported last year that the United States, together with Israel, was responsible for Stuxnet, the virus used to destroy centrifuges in an Iranian nuclear facility in 2010.


“It’s a bit of a grudge match,” said Mr. Lewis of the Center for Strategic and International Studies.

News Analysis: Obama Nominees in Step on Light Footprint

WASHINGTON — With the selection of a new national security team deeply suspicious of the wisdom of American military interventions around the world, President Obama appears to have ended, at least for the moment, many of the internal administration debates that played out in the Situation Room over the past four years.




He has sided, without quite saying so, with Vice President Joseph R. Biden Jr.’s view — argued, for the most part, in the confines of the White House — that caution, covert action and a modest American military footprint around the world fit the geopolitical moment. The question is whether that approach will fit the coming challenges of stopping Iran’s nuclear program and the potential collapse of Syria.


Gone for the second term are the powerful personalities, and more hawkish voices, that pressed Mr. Obama to pursue the surge in Afghanistan in 2009, a gamble championed by Secretary of State Hillary Rodham Clinton and Robert M. Gates, the former secretary of defense. Gone from the C.I.A. is the man who urged Mr. Obama to keep troops there longer, David H. Petraeus.


The new team will include two Vietnam veterans, Senator John Kerry and Chuck Hagel, who bear the scars of a war that ended when the president was a teenager, and a counterterrorism chief, John O. Brennan, who helped devise the “light footprint” strategy of limiting American interventions, whenever possible, to drones, cyberattacks and Special Operations forces. All are advocates of those low-cost, low-American-casualty tools, and all have sounded dismissive of attempts to send thousands of troops to rewire foreign nations as wasteful and ill-conceived.


Most important to Mr. Obama and his national security adviser, Tom Donilon, all three are likely to accommodate themselves, in ways their predecessors often did not, to a White House that has insisted on running national security policy from the West Wing.


“One of the characteristics of this administration has been that decision-making has been centered in the White House,” said Dennis B. Ross, a Mideast expert who left the Obama administration a year ago but never wandered far from some of its key debates. “And most second-term administrations don’t change their sociology.”


But if they grab hold of the national security levers after what many predict will be, for Mr. Hagel and Mr. Brennan, bruising confirmation hearings, they will confront problems that may test whether the light footprint carries enough weight.


“Issues 1 and 2 will be cutting the defense budget and confronting Iran,” said Michael Mandelbaum, a political scientist whose 2010 book, “The Frugal Superpower,” dealt with the challenge of trying to manage the world on the cheap. “And then you will have issues like Syria, which test the question of whether you can manage to control a dangerous situation with no boots on the ground — and unless something dramatic changes, there will be no boots.”


Mr. Hagel, who was both a senator and a cellphone entrepreneur, has long been a critic of Pentagon bloat. But others with business experience, like Donald Rumsfeld, have believed they could bring market discipline to one of the country’s most sprawling enterprises, only to discover that killing off unneeded weapons systems has almost nothing to do with business decisions and everything to do with the politics of Congressional districts and campaign funds.


Mr. Obama’s bet was that by appointing a Republican, he will better his chances of overcoming those obstacles. What he discovered even before announcing Mr. Hagel’s appointment is that the former senator burned many bridges with his Republican colleagues, in part with his outspoken opposition to the Iraq war, despite voting in 2002 to authorize military action, and to the 2008 surge when President George W. Bush was still in office.


“If the president thinks Chuck Hagel can get him the Republican votes to downsize the Pentagon,” said one former senior aide to Mr. Bush, who declined to speak on the record, “I think he is in for a very rude surprise.”


Then there is Iran, which will be a test for all three men, for different reasons. Mr. Hagel has been particularly vocal about the dangers of a military confrontation with Tehran. While both Mr. Gates and his successor, Leon E. Panetta, expressed similar concerns at various points in the first term, Mr. Hagel’s view is considerably to the left of Mr. Obama’s. The president has, gradually, endorsed “coercive diplomacy,” telling the American Israel Public Affairs Committee in March, “As I’ve made clear time and again during the course of my presidency, I will not hesitate to use force when it is necessary to defend the United States and its interests.”


But Mr. Hagel has opposed unilateral sanctions and suggested that threatening Iran just closes down opportunities for dialogue.


“The key to coercive diplomacy is that the side you are trying to influence is convinced you are willing to follow through on the threat,” said Mr. Ross, who drafted some of those threats. “The president has been clear, but from others there have been mixed messages.”

Mobile Apps Drive Rapid Changes in Search Technology





SAN FRANCISCO — When the Federal Trade Commission decided last week to close its antitrust investigation of Google without charges, one important factor, though hardly mentioned, was just beneath the surface: the mobile revolution.




Google has repeatedly made the argument — and the commission agreed — that the speed of change in the technology industry made it impossible for regulators to impose restrictions without stalling future innovations.


Exhibit A is the mobile device. Nowhere has technology changed as rapidly and consumer behavior as broadly. As people abandon desktop computers for mobile ones, existing tech companies’ business models are being upended and new companies are blooming.


“Mobile is very much a moving target,” said Herbert Hovenkamp, a professor of antitrust law at the University of Iowa who has been a paid adviser to Google. “This is a market in which new competitors come in a week’s time.”


When the commission began its investigation 19 months ago, for instance, the iPhone did not have the Siri voice search, Apple did not have its own mapping service and Yelp’s mobile apps had no ads. By the time the inquiry concluded, all of that had changed. Google had new competitors on all sides trying to chip away at its hold on the mobile search and advertising market.


Still, Google is even more dominant on mobile phones than on desktop computers. It has 96 percent of the world’s mobile search market, according to StatCounter, which tracks Web use. It collects 57 percent of mobile ad revenue in the United States, while Facebook, its nearest competitor, gets just 9 percent, according to eMarketer.


But, analysts say, as people change their search habits on mobile devices — bypassing Google to go straight to apps like Yelp’s, for example — that dominance could wane, or a competitor could swoop in and knock Google off its perch.


“It’s important to recognize that many mobile apps are really vertical search engines,” said Rebecca Lieb, a digital media analyst at the Altimeter Group. “It is impossible to really say anyone dominates a section of mobile in a secure way right now.”


On cellphones or tablets, for instance, people increasingly skip Google altogether in favor of apps like Flixster for movie times or Kayak for flights.


Apple is taking on mobile search with Siri on the iPhone, which can answer questions about the weather or search for nearby restaurants. With its new mapping service, Apple has also entered local search.


On Friday, Blekko, a search start-up, introduced an app called Izik for Apple and Android devices. It tries to make searching more tablet-friendly by showing images instead of just links, and making it easier to swipe through many pages of results with a finger.


On mobile devices, said Rich Skrenta, chief executive of Blekko, “the user experience is so different that we think it opens things up. On your desktop, if it doesn’t look like Google, you think that’s not a search engine. On a tablet, it’s just vastly different.”


Jon Leibowitz, chairman of the F.T.C., said at a news conference Thursday that the speed of change in the tech industry meant that “you want to be careful before you apply sanctions.”


The commission also considered Google’s partnerships with cellphone makers like Samsung and HTC that license Google search on phones, so that a search box shows up on the home screen. In the end it decided not to take action against Google.


Some Google critics said that even though the competitive landscape is different on mobile devices, it should not have influenced the government’s analysis of Google’s behavior on the desktop Web.


“There’s no doubt that mobile applications, including Yelp’s, give consumers the ability to bypass the major search engines and go directly to the best provider of the service they’re looking for,” said Vince Sollitto, vice president for government relations at Yelp. Still, he added, “I don’t see how that impacts how someone is acting anticompetitively on the desktop.”


(One of Google’s concessions to the federal agency, that it would allow other Web companies to ask Google not to show their content in its own vertical search products — a chief complaint of Yelp’s — applies to mobile as well.)


But others said antitrust enforcement in the 21st century needs to be more agile.

The New Old Age Blog: Who Should Receive Organ Transplants?

Joe Gammalo had been contending with pulmonary fibrosis, a scarring of the lungs, for more than a decade when he came to the Cleveland Clinic in 2008 seeking a lung transplant.

“It had gotten to the point where I was on oxygen all the time and in a wheelchair,” he told me in an interview. “I didn’t expect to live.”

Lung transplants are a dicey proposition, involving a huge surgical procedure, arduous follow-up, the lifelong use of potent immunosuppressive drugs and high rates of serious side effects. “It’s not like taking out an appendix,” said Dr. Marie Budev, the medical director of the clinic’s lung transplant program.

Only 50 to 57 percent of all recipients live for five years, she noted, and they will still die of their disease. But there’s no other treatment for pulmonary fibrosis.

Some medical centers would have turned Mr. Gammalo away. Because survival rates are even lower for older patients, guidelines from the International Society for Heart and Lung Transplantation caution against lung transplants for those over 65, though they set no age limit.

But “we are known as an aggressive, high-risk center,” said Dr. Budev. So Mr. Gammalo was 66 when he received a lung; his newly found buddy, Clyde Conn, who received the other lung from the same donor, was 69.

You can’t mistake the trend: A graying population and revised policies determining who gets priority for donated organs, have led to a rising proportion of older adults receiving transplants.

My colleague Judith Graham has reported on the increase in heart transplants, but the pattern extends to other organs, too.

The number of kidney transplants performed annually on adults over 65 tripled between 1998 and last year, according to data from the Scientific Registry of Transplant Recipients. In 2001, 7.4 percent of liver transplant recipients were over 65; last year, that rose to 13 percent.

The rise in elderly lung transplant candidates is particularly dramatic because, since 2005, a “lung allocation score” puts those at the highest mortality risk, rather than those who’ve waited longest, at the top of the list.

In 2001, about 3 percent of those on the wait list and of those transplanted were over 65; last year, older patients represented almost 18 percent of wait-listed candidates and more than a quarter of transplant recipients. (Medicare pays for the surgery, though patients face co-pays and considerable out-of-pocket costs, including for drugs and travel.)

The debate has grown, too: When the number of adults awaiting transplants keeps growing, but organ donations stay flat, is it desirable or even ethical that an increasing proportion of recipients are elderly?

Dr. Budev, who estimated that a third of her program’s patients are over 65, votes yes. As long as a program selects candidates carefully, “how can you deny them a therapy?” she asked. So the Cleveland Clinic has no age limit. “We feel that everyone should have a chance.”

At the University of Michigan, by contrast, the age limit remains 65, though Dr. Kevin Chan, the transplant program’s medical director, acknowledged that some fit older patients get transplanted.

“You can talk about this all day — it’s a tough one,” Dr. Chan said. Younger recipients have greater physiologic reserve to aid in the arduous recovery; older ones face higher risk of subsequent kidney failure, stroke, diabetes and other diseases, and, of course, their lifespans are shorter to begin with.

Donated lungs, fragile and prone to injury, are a particularly scarce commodity. Last year, surgeons performed 16,055 kidney transplants, 5,805 liver transplants and 1,949 heart transplants. Only1,830 patients received lung transplants.

“What if there’s a 35-year-old on a ventilator who needs the lung just as much?” Dr. Chan said. “Why should a 72-year-old possibly take away a lung from a 35-year-old?” Yet, he acknowledged, “it’s easy to look at the statistics and say, ‘Give the lungs to younger patients.’ At the bedside, when you meet this patient and family, it’s a lot different.”

These questions about who deserves scarce resources — those most likely to die without them? or those most likely to live longer with them? — will persist as the population ages. They’re also likely to arise when the International Society for Heart and Lung Transplantation begins working towards revised guidelines this spring. (I’d also like to hear your take, below.)

Lots of 65- and 75-year-olds are very healthy. Yet transplants themselves can cause harm and there’s no backup, like dialysis. Without the transplant, they die. But when the transplant goes wrong, they also die.

More than four years post-transplant, the Cleveland Clinic’s “lung brothers” are success stories. Mr. Conn, who lives near Dayton, Ohio, can’t walk very far or lift more than 10 pounds, but he works part time as a real-estate appraiser and enjoys cruises with his wife.

Mr. Gammalo, a onetime musician, has developed diabetes, like nearly half of all lung recipients. But he went onstage a few weeks back to sing “Don’t Be Cruel” with his son’s rock band, “a highlight of both our lives,” he said.

Yet when I asked Mr. Conn, now 73, how he felt about having priority over a younger but healthier person, he paused. “It’s a good question,” he said, to which he had no answer.
